If a birth occurred within the Town of Smithtown, a certified copy may be obtained from the Registrar located in the Town Clerk's Office.

A certified copy of a birth certificate may be issued only:
1. To a person with a New York State Court Order;
2. To the person named on the birth certificate, if 18 years of age or older;
3. To the parents of the person named on the birth certificate;
4. To the lawful representative of the person named or the parents of the person named on the birth certificate;
5. To a municipal, state or federal agency when needed for official purpose.

Naturalization Papers
Military ID
Employer’s ID
Police Report of Lost or Stolen ID
If the applicant's last name on the identification differs from the information on the certificate, a copy of the applicant's birth certificate, marriage license or proof of a legal name change must be provided.
COPIES CANNOT BE ISSUED UNLESS ONE OF THE ABOVE FORMS OF IDENTIFICATION IS PRESENTED ALONG WITH SUPPORTING DOCUMENTS IF NECESSARY.
A request can be made in person or to request by mail, fill out the application form, enclose a copy of one of the acceptable forms of ID listed above along with a money order made payable to the Town of Smithtown.
FEE: $10.00 per Transcript
Personal checks are not accepted.
